We really look forward to having you visit our office. There are several forms below that are helpful if you print out, pre-fill, and have ready for you at the time of your visit.
For your fist visit, please budget 5 or 10 minutes of extra time if you have not already filled out your paperwork. If you have filled out your paperwork, you can just show up on time.
Please bring the current eye drops and glasses you are using.
Please keep in mind that we can sometimes run behind, as not every patient is the same in terms of their assessment or time required for explanation. Therefore, we can understand that you may be a few minutes late, especially when trying to find our office for the first time.
When scheduling your appointment, we try to make sure we inform you of the appropriate type of insurance that covers your visit. However, sometimes people are scheduled under the wrong insurance for their visit type and we will have to explain that to you at your visit. Please keep in mind that their are differences between vision insurance and medical insurance. We have created a handout for your reference on the topic (please click this text).
Some Insurance basics about your visit.
We will bill your insurance. If you do not have insurance coverage; payment will be expected at time of service.
We are unable to bill out of the country insurances
All co-pays are due at the time of your visit. We are legally obligated to collect your co-pays.
We will call and obtain insurance eligibility and coverage benefits prior to your appointment.
